The applicant on this application agrees that the Richmond Hill Ultimate Club (RHUC) shall not be held responsible for any injury, loss, or damage whatsoever occurring to the participant while he/she is in attendance at RHUC activities or including while he/she is within the premises occupied by RHUC. I acknowledge my acceptance of the terms of this agreement by checking the box below. I consent to my attendance on the terms of this waiver of liability. I give RHUC permission to take pictures and/or videos of me playing to use for promotional purposes. I have read and agree to play by and adhere to the RHUC league rules outlined on the league website.
